As an RN Health Coach/Diabetes Educator youll partner with our primary care team to support patients managing diabetes and related chronic conditions. Youll have the opportunity to build ongoing relationships provide meaningful education and help patients turn clinical recommendations into realistic changes they can sustain.

About the Job

The RN Health Coach/Diabetes Educator works closely with our primary care team to help patients better understand and manage their health.

Youll receive referrals from providers and work primarily with patients managing diabetes and related comorbidities such as obesity hypertension and cardiovascular risk factors. Youll combine your nursing expertise with education motivational interviewing and individualized goal setting to help patients build the knowledge and confidence to manage their health successfully.

Beyond one-on-one coaching youll have opportunities to support broader wellness and engagement efforts through educational presentations webinars and onsite events.

What Youll Do

• Provide individualized diabetes education chronic disease management and health coaching

• Assess patient health needs risk factors medical history medications and relevant lab results

• Partner with patients to develop personalized care plans and meaningful achievable goals

• Educate patients on nutrition physical activity weight management stress sleep medication adherence and other lifestyle factors

• Use motivational interviewing and evidence-based strategies to encourage sustainable behavior change

• Help patients strengthen their self-management skills and take an active role in their health

• Monitor progress identify barriers and adjust coaching strategies as patient needs evolve

• Conduct telephonic and electronic outreach to at-risk patients to increase engagement with available care and resources

• Collaborate with providers and other members of the interdisciplinary care team

• Support wellness initiatives educational presentations webinars and patient engagement events

What Youll Bring

• Active unrestricted Registered Nurse (RN) license in Oklahoma

• Minimum of 2 years of experience in chronic disease management lab interpretation and/or health education; 35 years of relevant experience preferred

• Certified Diabetes Care and Education Specialist (CDCES) preferredor willingness to pursue certification

• Bachelors degree preferred

• Experience with or strong knowledge of motivational interviewing

• Passion for diabetes education preventive health and helping patients make sustainable lifestyle changes

• Confidence educating patients and presenting health information to individuals and groups

• Strong communication collaboration organization and problem-solving skills

• Ability to work independently while partnering effectively with an interdisciplinary care team

• Ability to travel within the Tulsa market as needed

• Willingness to obtain additional state RN licensure in the future if needed to support virtual services
