We are partnering with a well-established 151-bed acute care hospital in beautiful Western Montana to identify an RN Director of Case Management. Reporting directly to the Chief Nursing Officer this leader will oversee all aspects of the hospitals Case Management department while collaborating with physicians nursing leadership and interdisciplinary teams to improve patient outcomes care coordination and discharge planning.

As the RN Director of Case Management you will provide strategic and operational leadership for the department while ensuring patients receive coordinated high-quality care throughout their hospitalization.

Key responsibilities include:

• Lead the daily operations of the Case Management department
• Develop and implement departmental goals aligned with organizational objectives
• Oversee patient progression discharge planning utilization management and care coordination
• Mentor coach and develop Case Management staff while fostering a collaborative culture
• Partner with physicians nursing leadership and ancillary departments to improve patient flow and outcomes
• Monitor quality metrics regulatory compliance and departmental performance
• Manage departmental budgets staffing productivity and operational initiatives
• Promote evidence-based practices and continuous process improvement
• Serve as a resource and advocate for patients families and the interdisciplinary care team

Required

• Current RN license
• Bachelors degree in Nursing
• Minimum of one year of hospital Case Management leadership experience
• Current BLS certification

Preferred

• Two to five years of hospital Case Management leadership experience
• Master of Science in Nursing (MSN)

• Salary: $112000$150000
• $20000 sign-on bonus (two-year commitment; taxable)
