As a Hospice RN Clinical Team Specialist youll contribute to our success in the following ways:

• Applies professional nursing judgment to evaluate hospice documentation for clinical appropriateness regulatory compliance and eligibility requirements;
• Conducts a comprehensive clinical review of the initial plan of care and collaborates with clinical team to ensure all orders goals and interventions are present.
• Serves as a subject matter expert in electronic medical records (EMR) and hospice platforms including SHP.
• Oversees workflow including but not limited to admission discharge and HOPE.
• Provides clinical consultation education and mentoring to clinical team members regarding best practices and compliance with regulatory requirements.
• Collaborates effectively with members of the interdisciplinary team to support quality patient care and operational excellence.
• Utilizes operational data to identify systemic trends develops recommendations and partners with leadership to implement process improvements

What is Required

• Graduate of an accredited Certificate Diploma Associate or Baccalaureate School of Nursing.

• Minimum of three to five (5) years relevant experience as a registered nurse in a Hospice environment; preferred experience in hospice quality activities.
• Current State License as a Registered Nurse.
• Must have a dependable vehicle a valid drivers license and current auto insurance in accordance with the laws of the State.

• 10% travel.

• Meets all applicable agency policies and procedures related to health screening and required testing.
