Founded in 1940 Dignity Health - Marian Regional Medical Center is a 191-bed acute care nonprofit hospital located in Santa Maria California. Serving over 250000 patients annually the hospital offers a full complement of services including Level II Trauma Center Level III NICU cancer care orthopedics and stroke care. As our Case Manager RN you will coordinate the care and services of patients across the continuum of illness; promote effective utilization; monitor health care resources; and assume a leadership role with the interdisciplinary team to achieve optimal outcomes through the tools of case management.
Every day you will perform a wide variety of high-level patient care activities to include admission assessment screen for appropriateness and development of a discharge plan. Timely and appropriate discharges will be accomplished by evaluating the patients post-discharge needs and the provision of services necessary to meet those needs.
To be successful in this role you must be knowledgeable in community resources and alternate care facilities.
- Utilizes the nursing process to provide safe therapeutic care for patients
- Assists with coordination of patient-care activities/scheduling. Collaborates/cooperates to assure effective consistent management of clients
Works collaboratively with patient and family to obtain desired patient outcomes
Obtains all required physician orders TARs and authorizations for all services
- Incorporates all State Federal and accreditation requirements into care management activities and documentation
- Provides patient and family teaching necessary to meet the needs of the patient and family
- Minimum of One (1) year of RN experience
- Registered Nurse: CA (RN:CA)
- Basic Life Support - CPR (BLS-CPR) within 30 days
- Drivers License: CA (AUTO:CA)
- Auto Insurance (AUTO-INS)
Preferred
- Hospice or Home Health experience
