Position Overview
We are seeking a dedicated RN Case Manager to join our hospice this role you will manage a caseload of 1015 patients coordinating care across an interdisciplinary team to ensure comfort dignity and quality of life.
This position offers a consistent MondayFriday schedule manageable caseload and a localized territory in the northwest metro allowing you to build meaningful relationships with patients and families.
What Youll Do
Patient Care & Case Management:
- Conduct initial and ongoing patient assessments
- Develop and manage individualized hospice care plans
- Administer medications and provide palliative interventions
- Educate patients and families on symptom management and care needs
- Provide emotional support to patients and families
Care Coordination:
- Manage and coordinate care for assigned patients
- Collaborate with physicians and interdisciplinary team members
- Participate in care conferences and family meetings
- Advocate for patient needs and ensure access to resources
Clinical & Operational Excellence:
- Maintain accurate and timely clinical documentation
- Supervise and support hospice aides
- Ensure necessary equipment and supplies are in place
- Participate in on-call rotation (5 days/month)
- Contribute to quality improvement initiatives
Qualifications
- Active RN license in Minnesota
- Graduate of an accredited School of Nursing
- CPR certification
- 1 year of acute care experience required
- Hospice or home care experience preferred
- Strong assessment and clinical decision-making skills
- Valid drivers license and insured vehicle
