Contract Duration: 13 Weeks
Schedule: Monday Friday 8:00 AM 5:00 PM
On-Call: Rotating on-call/weekend coverage
Pay Rate: $55.00 $62.00/hour (Based on Traveler Experience & Assignment Details)

Job Overview:
MTK Healthcare INC. is seeking a compassionate and experienced Registered Nurse (RN) Case Manager for a Hospice assignment with HopeWest Hospice in Montrose CO. This role is ideal for nurses passionate about providing meaningful patient-centered care while supporting patients and families through their healthcare journey.

The Hospice RN Case Manager provides compassionate care clinical expertise and guidance to patients and caregivers while managing an assigned caseload and leading interdisciplinary care teams.

Position Responsibilities:
Provide skilled nursing care to hospice patients in private homes nursing homes group homes and assisted living facilities
Perform comprehensive patient assessments and develop individualized care plans
Manage assigned patient caseload and coordinate care with interdisciplinary teams
Educate patients and caregivers on disease processes symptom management and goals of care
Provide emotional support and guidance to patients and families
Utilize strong clinical judgment and critical thinking skills to manage complex patient needs
Serve as a patient advocate while promoting dignity comfort and quality of life
Collaborate with physicians and healthcare professionals to ensure effective care delivery
Provide symptom management and end-of-life care support
Participate in rotating on-call and weekend coverage as needed

Required Qualifications:
Active Registered Nurse (RN) License
Hospice Home Health or Case Management experience preferred
Strong assessment communication and critical thinking skills
Ability to work independently while collaborating with a multidisciplinary team
Commitment to compassionate and patient-focused care
