Job Summary

Job Title: RN Case Manager (Acute Care)
Location: Eureka CA
Type: Contract
Duration:13 Weeks
Shift:Day Shift Hours: 8:00 AM 4:30 PM Schedule: 5 days/week Hours/Week: 40
Pay Rate:
Local Pay: $60/hr (Based on experience)
Travel Pay: $66/hr including stipend (Based on experience)

Position Overview:
Navitas Healthcare LLC is seeking a Registered Nurse Case Manager for an exciting Travel or Local job in Eureka CA.

Key Responsibilities

  • Perform case management duties for acute care patients including assessment planning and coordination of care.
  • Develop individualized care plans in collaboration with physicians nurses and interdisciplinary teams.
  • Facilitate patient transitions including discharge planning referrals and follow-up care coordination.
  • Communicate with patients families and care providers regarding treatment plans and care needs.
  • Ensure compliance with facility policies procedures and regulatory standards.
  • Participate in unit assignments and float between units as needed to support patient care.
  • Maintain accurate and timely documentation in the Epic electronic medical record system.
  • Assist patients with resources and services including housing support for travelers if required.

Required Qualifications

  • Active Registered Nurse (RN) license in California.
  • Minimum of 2 years of acute care nursing experience (first-time case managers may be considered with clinical approval).
  • Experience in case management or care coordination preferred.
  • Strong communication organizational and critical thinking skills.
  • Ability to work every other weekend as scheduled.
  • Proficiency with electronic medical records preferably Epic.
  • Ability to float between units based on staffing needs.
