RN Admissions Nurse, Hospice

As an Admissions Nurse for our Hospice team you will:

• Traveling to CHLs and homes to see our hospice clients and their families
• Complete initial resumption follow up a significant change of status and discharge assessments including comprehensive head to toe assessment HIS assessment and all associated assessments with admission or discharge
• Initiate an individualized plan of care and update the plan of care for resumptions significant change in status and re-certifications.
• Use health assessment data to determine nursing diagnosis
• Obtain implement perform and maintain orders as received by physicians overseeing patient care services
• Initiate appropriate preventive and rehabilitative nursing procedures. Administer medications and treatments as prescribed by the physician
• Counsel the patient and family in meeting nursing and related needs
• Provide health care instructions to the patient as appropriate per assessment and plan of care
• Assist the patient with activities of daily living and facilitates the patients efforts toward self-sufficiency and optional comfort care.
