We are seeking an experienced and compassionate Registered Nurse (RN) to join our Care Management team as a Remote RN Admissions Coordinator. This position is primarily home-based with approximately 80% of work performed remotely and up to 20% local travel within Sedgwick County to support provider relationships care coordination meetings and other business needs.
The RN Admissions Coordinator is responsible for coordinating admissions to long-term care skilled nursing rehabilitation and post-acute care settings for members requiring ongoing services. The ideal candidate has experience in home health long-term care rehabilitation managed care or admissions coordination and enjoys working independently while collaborating with interdisciplinary teams.
Coordinate admissions and transitions of care for members entering long-term care skilled nursing facilities (SNFs) rehabilitation centers or other post-acute settings
Review clinical documentation and assessments to determine appropriate levels of care and placement
Serve as the primary liaison between the health plan hospitals facility admissions teams providers discharge planners and case managers
Coordinate prior authorizations and ensure services meet clinical and coverage guidelines
Collaborate with Care Management and Utilization Management teams to promote timely appropriate placements
Educate members and families regarding available services benefits and care transition expectations
Maintain accurate documentation within electronic medical record and care management systems
Ensure compliance with HIPAA company policies and applicable state and federal regulations
Travel locally within Sedgwick County as needed for provider meetings care coordination activities and other business-related responsibilities (approximately 20%)
$40.00 per hour

Active unrestricted Registered Nurse (RN) license in the State of Kansas
Minimum of three years of clinical experience in home health long-term care rehabilitation admissions utilization management or care coordination
Strong understanding of post-acute care services and transitions of care
Excellent communication organizational and critical thinking skills
Experience using electronic medical records (EMR) and care management software
Ability to work independently in a remote environment while managing multiple priorities
Valid drivers license and reliable transportation for local travel within Sedgwick County
Managed care health plan or insurance experience
Utilization Management or Case Management experience
Skilled nursing facility or rehabilitation admissions experience
Experience coordinating post-acute care transitions
Bilingual English/Spanish is a plus but not required
This position is designed to be primarily remote with approximately 80% of work performed from a home office. Occasional local travel (up to 20%) throughout Sedgwick County is required to support providers attend meetings and facilitate care coordination activities.
