What Youll DoAs a Senior Services Coordinator youll serve as the primary resource connector and advocate for residents across multiple CHP communities by:
- Conducting resident assessments and developing individualized support plans
- Connecting residents with healthcare benefits transportation food assistance and other essential services
- Planning and facilitating wellness educational and social programs
- Building partnerships with community organizations and service providers
- Assisting residents with benefit applications and service navigation
- Maintaining accurate documentation reporting and compliance requirements
- Managing program resources and supporting community operations
- Collaborating with property management and internal teams to improve resident outcomes

Qualifications- High school diploma or equivalent and at least two years of experience in human services social services case management resident services or a related field; OR
- Bachelors degree in human services social work sociology psychology gerontology or a related field with at least one year of relevant experience
- Minimum one year of direct service experience supporting seniors and/or individuals with disabilities or special needs
- Valid drivers license meeting CHP insurability requirements

- Data tracking and reporting systems such as Smartsheet
Technical Skills- Microsoft Office Suite (Word Excel Outlook PowerPoint)

- Online benefits and government service systems
- Documentation data entry and reporting tools
