Job Overview
We are seeking a Rehabilitation Manager to oversee therapy services within a PACE (All-Inclusive Care for the Elderly) program. This role is responsible for coordinating rehabilitation operations supervising therapy staff and ensuring high-quality compliant care delivery for elderly participants with complex needs.
The Rehabilitation Manager will provide clinical leadership operational oversight and strategic guidance to ensure effective therapy program delivery across multiple care settings including home-based services.
Key Responsibilities
Oversee rehabilitation therapy programs and ensure alignment with clinical standards
Supervise and mentor rehabilitation staff (PT OT and related therapy teams)
Coordinate participant care plans in collaboration with interdisciplinary teams (IDT)
Ensure compliance with state federal and PACE regulatory requirements
Support business planning budgeting and operational decision-making
Conduct home visits to assess therapy needs and equipment setup when required
Collaborate with clinical leadership to identify staffing and program needs
Oversee contract management for therapy services where applicable
Ensure high-quality patient-centered rehabilitation services
Requirements
Bachelors Degree in Physical Therapy or Occupational Therapy required
Active PT or OT license (California preferred)
Minimum 5 years rehabilitation experience working with complex or elderly populations
Minimum 2 years leadership or supervisory experience in a rehab setting
Strong knowledge of therapy regulations and compliance standards
CPR and First Aid certification (required within 180 days of hire preferred at start)
Strong leadership communication and team development skills
Ability to travel occasionally for home-based care oversight
