What Youll Do

• Provide skilled nursing visits (RNs/LPNs) based on the clients plan of care.
• Medication management vitals treatments wound care education and assessments.
• Collaborate with our clinical team for changes in condition and care coordination.
• Document visits electronically (user-friendly system; training provided).

• Active Indiana RN or LPN license.
• Reliable transportation and willingness to travel locally.
