Contract: 13 weeks
Schedule: Saturday and Sunday 7:00 AM 11:00 PM
Total Working Hours per week: 40 hours
Experience: Minimum 1 year

About the Role
We are seeking a Weekend Field Registered Nurse for a 13-week assignment in Battle Creek Michigan. This role focuses on participant assessments home visits wound care health assessments care coordination and timely response to participant needs in a PACE care setting.

Responsibilities
Conduct in-person participant assessments to determine level of care needs
Complete regular field and home visits for nursing care and treatment
Provide wound care IV hydration lab draws assessments and other nursing tasks
Respond promptly to calls from providers triage and support staff
Document calls visits follow-ups incidents and care updates accurately
Coordinate with providers staff pharmacies DME suppliers facilities and family members
Initiate emergency treatment when needed and escalate concerns appropriately

Requirements
Active Michigan Registered Nurse license required
Associate degree or nursing diploma required; BSN preferred
CPR certification through AHA required or must be willing to obtain
Minimum 1 year of experience with frail elderly or long-term care population
Home care experience preferred
Wound care experience preferred
Reliable transportation valid Michigan drivers license and car insurance required
TruChart experience preferred
Must be comfortable working weekends and field-based visits
