Developmental Disabilities & Residential Care

ATC Healthcare is seeking an experienced and compassionate Registered Nurse (RN) Case Manager to provide nursing oversight care coordination and clinical support for individuals with intellectual and developmental disabilities (I/DD) residing in group homes and other residential care settings.

The RN Case Manager will be responsible for assessing residents healthcare needs developing and monitoring individualized care plans coordinating medical services and ensuring the delivery of safe high-quality person-centered care.

The ideal candidate will have experience working with individuals with developmental disabilities strong clinical assessment skills and the ability to collaborate effectively with Direct Support Professionals (DSPs) physicians families and interdisciplinary healthcare teams.

• Conduct comprehensive nursing assessments of residents with intellectual and developmental disabilities.

• Develop implement and regularly update individualized nursing care plans.

• Monitor residents medical conditions treatment plans and overall health status.

• Coordinate routine medical appointments specialty consultations and follow-up care.

• Identify changes in residents conditions and implement appropriate nursing interventions.

• Review hospital discharge instructions and coordinate transitions of care.

• Maintain accurate and timely clinical documentation nursing assessments and progress notes.

• Oversee medication administration practices to promote resident safety and compliance with applicable regulations.

• Review physician orders medication administration records (MARs) and treatment plans.

• Monitor medication effectiveness adverse reactions and potential medication errors.

• Coordinate medication changes and prescription renewals with physicians and pharmacy providers.

• Provide medication administration training and supervision to authorized staff as permitted by applicable regulations.

• Ensure appropriate documentation of medications treatments and nursing interventions.

• Provide clinical guidance and support to Direct Support Professionals (DSPs) and other residential staff.

• Train staff on residents medical conditions individualized care plans and health-related needs.

• Educate staff on infection prevention emergency procedures and recognition of changes in resident condition.

• Monitor staff adherence to nursing care plans and established healthcare protocols.

• Collaborate with residential managers and program leadership to address clinical concerns and promote quality care.

• Collaborate with physicians families guardians therapists care managers and other healthcare professionals.

• Participate in interdisciplinary team meetings and individualized service planning.

• Maintain compliance with applicable state and federal regulations agency policies and facility procedures.

• Support regulatory surveys clinical audits and quality assurance initiatives.

• Ensure appropriate reporting and documentation of medical incidents and changes in resident health status.

• Promote residents rights dignity independence and overall quality of life.

• Active unrestricted Registered Nurse (RN) license in the state of practice.

• Graduate of an accredited nursing program; BSN preferred.

• Minimum of one year of nursing experience.

• Previous experience in case management group homes developmental disabilities residential care or community-based nursing preferred.

• Experience supervising or providing clinical guidance to DSPs and other direct care staff preferred.

• Knowledge of medication administration nursing assessments care planning and clinical documentation.

• Familiarity with OPWDD regulations and AMAP requirements preferred for New York assignments.

• Strong organizational communication critical-thinking and problem-solving skills.

• Ability to work independently and collaborate with interdisciplinary healthcare teams.

• Current CPR/BLS certification as required by the assignment.

• Ability to travel between residential facilities or group homes as required.

• Must successfully complete applicable background checks health screenings and credentialing requirements.
