As an AmbulatoryRN you willprovide clinical triage patient assessment education care coordination andongoing follow-up. Youll manage patientportal communicationsconduct nurse visits support treatment plans and serveas a trustedresource for patientsthroughout their carejourney. The idealcandidate is anexcellent communicatorwho thrives ina collaborative patient-centeredenvironment and iscommitted to deliveringexceptional ambulatorycare.

• Current Massachusetts RegisteredNurse (RN) license.
• Current AHA BLS certification.
• Minimum of two(2) years ofRN experience.
• Strong assessment triage patient education andcare coordination skills.
• Excellentverbal written andinterpersonal communicationskills.
• Demonstrated ability tocollaborate effectivelywith providers andinterdisciplinary teams.
• Commitment to deliveringcompassionate patient-centered care toindividuals from diversebackgrounds.
• Ambulatory or specialtypractice nursing experience.
• Experience caring forpatients with chronicor complex medicalconditions.
• Adult and pediatricnursing experience ora willingness towork with pediatricpatients.
