Job Summary

Position:Registered Nurse (RN)

Department: Community-Based IDD Nursing

Location: 403 Rector Street

Hourly Rate: $42.00 per/hour

Hours: 37.5 hours per week (Monday Friday) Rotating On-Call Coverage

Are you looking for a nursing role where you can build meaningful relationships support individuals in the community and make a lasting impact We are seeking a compassionate and organized Registered Nurse to support adults with intellectual and developmental disabilities living in community homes throughout Philadelphia. This is an excellent opportunity for nurses who enjoy care coordination health education advocacy and building long-term relationships with the people they serve.

What Youll Do

Visit residents in community homes and monitor their health needs

Review medical appointments physician orders and treatment plans

Coordinate care with physicians specialists pharmacies and support teams

Train staff on health-related supports and medical changes

Participate in interdisciplinary meetings and care planning

Support hospital admissions discharges and follow-up care

Complete nursing documentation and required assessments

Participate in a rotating on-call schedule

Qualifications

Current Pennsylvania LPN License

Two years of nursing experience preferred

Valid drivers license and reliable transportation

Ability to obtain required background clearances
