Registered Nurse RN On-Call

• Provide on-call nursing coverage for one designated week each month including evenings weekends and holidays as scheduled.
• Respond promptly to patient caregiver and staff calls regarding changes in condition symptom management medication concerns and care needs.
• Perform telephone triage and determine appropriate interventions based on patient needs.
• Conduct skilled nursing visits when clinically indicated.
• Coordinate care with physicians case managers therapists hospice staff and other healthcare professionals.
• Assess implement and evaluate individualized patient care plans.
• Administer medications and monitor for effectiveness and adverse reactions as needed.
• Educate patients and caregivers regarding disease processes medication management safety symptom control and available resources.
• Document assessments interventions communications and changes in patient condition accurately and timely.
• Report significant findings and concerns to the appropriate provider and supervisor.
• Maintain compliance with agency policies Medicare Conditions of Participation and professional nursing standards.

• Current active and unrestricted Utah Registered Nurse (RN) license.
• Home Health and/or Hospice experience preferred.
• Excellent assessment critical thinking and clinical decision-making skills.
• Strong communication and interpersonal skills.
• Ability to work independently and manage urgent situations effectively.
• Reliable transportation valid drivers license and current auto insurance.
• Current CPR certification required.
• Proficiency with electronic medical records and documentation systems preferred.
• Ability to travel throughout Utah County and Salt Lake County as needed.

• On-Call Coverage: 7 days on / 7 days off
• Additional visit opportunities may be available based on agency needs and employee preference.
