As a Home Health Registered Nurse youll provide personalized one-on-one care that helps patients recover from illness manage chronic conditions and maintain their independence while receiving care in the comfort of their own homes.

- Perform comprehensive nursing assessments and skilled visits
- Complete Start of Care (SOC) Resumption of Care (ROC) Recertification and Discharge assessments
- Develop and implement individualized plans of care
- Provide skilled nursing services including medication management wound care disease management IV therapy as appropriate and patient education
- Monitor patient progress and communicate changes with physicians and the interdisciplinary care team
- Coordinate care with therapists social workers and home health aides
- Complete timely and accurate clinical documentation using WellSky
- Educate patients and caregivers to promote safety independence and improved health outcomes
- Active Maryland Registered Nurse (RN) License Required
- Valid Drivers License and Reliable Transportation
- Strong Clinical Assessment Critical Thinking and Documentation Skills
- Home Health Experience Preferred but Not Required
- OASIS Experience Preferred
- Experience with WellSky Home Health is a plus
