What Youll Do

• Coordinate the Resident Assessment Instrument (RAI) process to ensure accurate and timely completion of resident assessments
• Ensure compliance with Medicare Medicaid OBRA and other payer program requirements
• Ensure MDS assessments accurately reflect each residents physical mental and psychosocial status
• Verify documentation supports services provided and reimbursement accuracy
• Support interdisciplinary collaboration for accurate care planning assessment completion and ongoing case management coordination
• Collaborate with clinical and administrative teams regarding admissions transitions of care and payer communication related to skilled nursing services
• Monitor assessment schedules and ensure regulatory timelines are met
• Assist with quality improvement initiatives and survey readiness efforts

Required Qualifications

• Graduate of an accredited school of nursing with a current RN license in the state of employment
• MDS 3.0 and case management experience in a skilled nursing or post-acute care setting required
• Ability to work independently while collaborating effectively with interdisciplinary teams
• Strong organizational communication and critical thinking skills
• Working knowledge of Microsoft Office applications
• Preferred Qualifications
• Previous long-term care or skilled nursing experience
• RAC-CT certification or willingness to obtain within six months of hire and maintain certification requirements
