• Provide skilled nursing care to patients in their homes in accordance with physician orders and individualized care plans
• Perform comprehensive assessments and ongoing evaluations of patients with acute illnesses chronic conditions injuries and post-surgical needs
• Develop implement and update patient care plans to promote optimal health outcomes
• Administer medications treatments wound care and other skilled nursing interventions
• Monitor patient progress identify changes in condition and communicate findings to physicians and interdisciplinary team members
• Educate patients and caregivers regarding disease management medications treatments nutrition and safety practices
• Coordinate care with physicians therapists social workers and other healthcare professionals
• Supervise and collaborate with Licensed Practical Nurses (LPNs) Home Health Aides (HHAs) and other support staff as appropriate
• Complete accurate and timely clinical documentation in accordance with agency policies and regulatory requirements
• Participate in case conferences quality improvement activities and discharge planning to ensure continuity of care

• Graduate of an accredited nursing program
• Active Registered Nurse (RN) license in the State of New Hampshire
• Minimum of one (1) year of nursing experience; home health hospice rehabilitation or medical-surgical experience preferred
• Current CPR certification
• Valid drivers license reliable transportation
• Knowledge of state and federal home health regulations and standards of practice
• Strong clinical assessment critical thinking and care coordination skills
• Excellent communication documentation and interpersonal skills
• Compassionate approach to patient-centered care and family education

• Previous home health experience
• Experience with electronic medical records (EMR)
• BSN preferred but not required
