As a Home Health Registered Nurse youll provide skilled individualized care to patients in rural communities helping them recover from illness manage chronic conditions prevent hospitalizations and remain safely in their homes.
Your care helps bridge the gap for patients who depend on experienced clinicians to bring quality healthcare directly to their doorstep.

- Perform comprehensive nursing assessments and skilled visits
- Complete Start of Care (SOC) Resumption of Care (ROC) Recertification and Discharge assessments
- Develop and implement individualized plans of care
- Provide skilled nursing services including medication management wound care disease management and patient education
- Monitor patient progress and communicate changes with physicians and the interdisciplinary care team
- Coordinate care with therapists social workers and home health aides
- Complete timely and accurate clinical documentation using WellSky
- Educate patients and caregivers to promote safety independence and improved health outcomes
- Active Virginia Registered Nurse (RN) License Required
- Valid Drivers License and Reliable Transportation
- Strong Clinical Assessment Critical Thinking and Documentation Skills
- Home Health Experience Preferred but Not Required
- OASIS Experience Preferred
- Experience with WellSky Home Health is a plus
- Passion for Delivering Exceptional Patient-Centered Care
