Registered Nurse (RN) Case Manager Hospice

Registered Nurse (RN) - Hospice

Shift Available: PRN

Service Areas for Coverage: Northwest Southwest Southeast

Responsibilities:

• Oversee nursing care team.
• Communicate and collaborate with physicians therapists and other healthcare professionals to ensure cohesive and comprehensive patient care.
• Plan organize and direct home care services.
• Conduct assessments of patient care needs and develop care plans.
• Educate patients and their families on managing their health conditions medication regimens and any necessary lifestyle changes.
• Prepare timely complete and accurate clinical notes.
• Night and Weekends are required per hospice needs of patients

Qualifications:

• Valid RN license in the State of Texas.
• Hospice Experience (at least 1 year).
• Knowledge of home health regulations and general healthcare compliance.
• Experience with Home Care Home Base (HCHB) EMR (preferred but not required).
• Excellent communication and people skills are essential.
• Ability to work independently and as part of a team.
