Perform supervisory visits for caregivers and clients
Monitor client health status and coordinate care needs
Provide caregiver training and clinical guidance when needed
Ensure compliance with state regulations and agency policies
Communicate with families physicians and care teams regarding client needs
Maintain accurate documentation and client records
Home care senior care hospice or community health experience preferred
Strong communication and assessment skills
Compassionate and professional bedside manner
Reliable transportation for occasional local client visits
Ability to work independently and manage flexible scheduling
