Requirements:

• Active Registered Nurse License in the state of Pennsylvania
• Tuberculosis (TB) screening/QuantiFERON/Chest X-Ray (within 1 year)
• Proof of U.S. work authorization (valid unexpired documents: government issued ID)
• Reliable transportation

Responsibilities:

• ConductingStart of Care (SOC)visits to evaluate medical physical and emotional needs and creating individualized care plans.
• Performingregular reassessmentsto monitor progress and adjust care plans as needed.
• Providingpatient and family educationon condition management medication adherence and preventive health measures.
• Collaborating withphysicians therapists social workers and familiesto ensure coordinated comprehensive care.
• Maintainingaccurate clinical documentation in compliance with Pennsylvania Board of Nursing regulations and agency policies.
• Supervising and supportingCertified Home Health Aides (CHHAs)and other care team members.
