This role blends clinical care patient education and case management.
Flexible Monday-Friday 8am-5pm type-of schedule. No Weekends No Holidays!
Will be covering census in one facility; but must be flexible to support in other surrounding home facility centers as needed including but not limited to: New London and Norwich. CT.
Excellent Paid Training Provided! Dialysis Experience is NOT required. Nephrology nursing provides a fresh lens and set of skills to those newly entering the field.
Key Responsibilities:
Train patients and caregivers on PD procedures and self-care
Manage a caseload of PD patients including assessments and care planning
Monitor treatment progress conduct home visits and provide on-call support
Collaborate with interdisciplinary teams to ensure safe high-quality outcomes
Schedule:
MondayFriday 8-hour shifts
Weekends and holidays off
Flexible scheduling to manage your own caseload
Requirements:
RN license in the state of practice
Willing to obtain both CT and NY State license (we reimburse)
Minimum of 12 months of RN experience
Valid drivers license insurance and reliable transportation
ADN required; BSN preferred
Basic computer skills (MS Word Outlook)
Preferred Experience:
Dialysis (PD HHD chronic or acute)
Patient education or teaching background
Case management or telehealth experience
CNN or CDN certification
