Registered Nurse Jacksonville IL
A healthcare organization is seeking a compassionate and dedicated Registered Nurse (RN) to provide skilled nursing care in a home-based setting. This opportunity is ideal for nurses who enjoy working independently while building meaningful relationships with patients and supporting their health and well-being.
Responsibilities:
Conduct patient assessments and develop individualized care plans
Provide direct nursing care and monitor patient progress
Administer medications and coordinate treatment plans as appropriate
Educate patients and caregivers on health conditions and care needs
Maintain accurate and timely clinical documentation
Collaborate with physicians and other healthcare professionals to support quality patient care
Follow company policies professional standards and regulatory requirements
Perform other duties as assigned

Qualifications:
Active Registered Nurse (RN) license required
Valid drivers license and reliable transportation required
Current CPR certification preferred
Previous nursing experience preferred
