Contract: 12 Weeks
Schedule: Monday Friday 8:00 AM 5:00 PM
Guaranteed Hours: 40 Hours/Week
Experience: Minimum 2 years
License: Active Colorado Registered Nurse license required
On-Call: Occasional weekend/back-up on-call may be required

About the Role

We are seeking a compassionate Registered Nurse Hospice Case Manager for a contract opportunity in Grand Junction CO. This role is ideal for a nurse who is passionate about hospice care patient advocacy symptom management and supporting patients and families during meaningful stages of care.

The nurse will manage an assigned hospice caseload and coordinate care across private homes nursing homes group homes and assisted living settings.

Responsibilities

Provide compassionate hospice nursing care to assigned patients
Assess patient needs and develop individualized care plans
Educate patients and caregivers on comfort care and symptom management
Coordinate with the interdisciplinary hospice care team
Support families with guidance emotional support and care planning
Monitor patient condition and update care plans as needed
Participate in occasional weekend/back-up on-call coverage

Requirements

Active Colorado Registered Nurse license required
Minimum 2 years of nursing experience required
Hospice case management or home health experience preferred
Strong assessment communication and critical-thinking skills
Ability to provide care in home-based and facility-based settings
Compassionate patient-centered approach to end-of-life care
