Park Nicollet is looking to hire a Homecare Coordinator to join our Homecare team!Come join us as a Partner for Good and help us make an impact on the care and experience that our patients and their families receive every day.
Position Summary:
The Homecare Coordinator is responsible for planning and coordinating the delivery of individualized care and comprehensive service to patients and families. The Homecare Coordinator supports the following major Homecare processes: admission discharge referral delivery of care complaints staff orientation training and development and billing.
Work Schedule:
M-F 8am-4:30pm every 6th-8th weekend.
Required Qualifications:
- Education Experience or Equivalent Combination:
- Minimum 3 years experience in home care
- Registered nurse: Associate or Diploma degree
- Licensure/ Registration/ Certification:
- Current license to practice as a professional nurse in Minnesota.
- Knowledge Skills and Abilities:
- Excellent written and oral communication skills and ability to speak to groups.
- Committed to the philosophy of customer service.
- Must possess knowledge of reimbursement regulations.
- Ability to problem solve and interface with primary customers.
- Ability to relate to allied health professionals and to work with and through others.
- Demonstrate knowledge of age-related growth and development principles necessary to provide appropriate services and assure safety of patients.
Preferred Qualifications:
- Education Experience or Equivalent Combination:
- PHN preferred.
- Previous coordinator/supervisory experience preferred.
