Contract: 13 weeks
Schedule: Day Shift Monday Friday; possible weekend shifts
Guaranteed Hours: 36 hours per week
Experience: Minimum 1 year of prior clinical experience required
About the Role
We are seeking a compassionate Home Health Registered Nurse for a 13-week contract opportunity in St. Johnsbury Vermont. This role provides professional nursing services to clients of all ages including admissions evaluations care planning and coordinated home-based care.
Responsibilities
Provide skilled home health nursing care to clients of all ages
Complete admissions patient evaluations and individualized care plans
Coordinate care with homecare professionals paraprofessionals patients and families
Monitor patient progress and document care accurately in Homecare Homebase
Support high-quality compassionate care across assigned home health visits
Requirements
active vermont registered nurse license required
minimum 1 year of prior clinical nursing experience required
bls certification required; acls pals and nrp may be preferred based on facility needs
home health experience and homecare homebase experience preferred
must be available for monday friday day shift with possible weekend coverage
