As a Registered Nurse Behavioral Health youll contribute to our success in the following ways:
- Assess patients physical emotional social and environmental needs.
- Manage a caseload and develop coordinate and update individualized care plans.
- Complete initial assessments and provide ongoing evaluations during patient visits.
- Monitor patient progress and communicate changes with physicians supervisors and the care team.
- Update care plans based on patients changing needs.
- Educate patients families and caregivers about the plan of care and behavioral health support.
- Teach behavioral health care and symptom management to staff in partner care settings.
- Supervise and support Behavioral Health Aides and LPNs while ensuring quality care.
- Report safety concerns or other issues to supervisors as needed.
- Provide compassionate end-of-life support and complete bereavement assessments.
What is Required
- Current State license as a Registered Nurse.
- Graduate of an accredited Certificate Diploma Associates or Baccalaureate School of Nursing.
- 1 year of experience as a nurse in a clinical care setting.
- Home Health experience is preferred but not required.
- Ability to frequently lift push pull and support up to 50 pounds including positioning or transferring patients and moving equipment
- Must possess a valid drivers license maintain an acceptable driving record OR have reliable transportation to travel between patient homes and assigned work locations.
