About the Role

We are seeking a Hospice Registered Nurse Case Manager for a 13-week contract opportunity in Montrose Colorado. This role provides compassionate hospice care to patients in their homes nursing homes assisted living facilities and other residential settings while supporting families and caregivers through education symptom management and care coordination.

Responsibilities

Manage an assigned hospice patient caseload and coordinate care with the interdisciplinary team
Assess patient needs monitor changes and provide symptom management support
Educate patients and caregivers on goals of care comfort measures and end-of-life support
Provide compassionate nursing care in homes nursing facilities group homes or assisted living settings
Document patient care accurately and support safe communication with families and care teams

Requirements

Active Colorado Registered Nurse License Required
Minimum 2 years of nursing experience required
Hospice home health case management or palliative care experience preferred
Strong assessment critical thinking symptom management and patient education skills required
Must be available for Monday Friday schedule with rotating on-call weekend and holiday coverage
