Registered Nurse (RN) Ellsworth AFB SD

Schedule: MondayFriday (No nights or weekends) 7AM-4:30PM
U.S. Citizenship Required

About the Role

Join the 28th Medical Group at Ellsworth Air Force Base working within a Patient-Centered Medical Home (PCMH) model that delivers coordinated team-based care to military members and their families.

In this role you will be part of a dedicated care teamincluding physicians nurse practitioners nurses medical technicians and behavioral health consultantsfocused on whole-person preventive and continuous care. The RN plays a key role in delivering direct patient care coordinating services and ensuring seamless referrals through the PCMH model while using digital tools to support patient engagement and follow-up.

Qualifications
  • Graduate of an accredited nursing program (ADN or higher)
  • Current active and unrestricted Registered Nurse license (South Dakota)
  • BLS certification (ACLS may be required depending on assignment)
  • Minimum 1 year of nursing experience required
Preferred Experience & Skills
  • At least 2 years of experience in outpatient ambulatory or primary care settings
  • Strong clinical skills with the ability to think critically in a fast-paced environment
  • Experience using electronic health records (EHR)
  • Excellent communication skills and a team-oriented mindset
  • Military healthcare experience preferred but not required
Key Responsibilities
  • Deliver patient care within a team-based primary care model
  • Perform assessments administer medications and provide treatments
  • Educate patients on health wellness and care plans
  • Coordinate care referrals and follow-ups
  • Ensure compliance with clinical standards and MTF policies
  • Respond to urgent and emergent patient needs
  • Supervise and delegate tasks as appropriate
Work Environment
  • 28th Medical Group Building 6000 Ellsworth AFB
  • Appointment-based primary care clinic
  • Collaborative care teams including behavioral health support
  • Use of digital tools for patient communication and care coordination
Additional Requirements
  • Must successfully complete a background investigation fingerprinting and a rigorous credentialing process to work on a military installation
  • Must meet all applicable federal contract and clinical competency requirements
