MANDATORY QUESTIONNAIRE *Please answer the following questions and send back ASAP. An offer cannot me made without these answers.* 1. Is your candidate flexible with scheduling and aware that the client has a zero-tolerance policy for tardiness and unexcused absences (Y or N) 3. Are there any barriers (ex. upcoming vacation) that would prevent your candidate from completing compliance on time The compliance deadline is 12 Noon EST the Wednesday prior to the start date. (Y or N) 4. Is your candidate aware of the Pass Fail EKG exam required by all RNs for Cone Health regardless of specialty (Y or N) 4x10s Days 7a-530p Monday - Friday with lates required a few times/month. STEMI call required. Approx. one week worth of call over a 12-16 week period. This includes 24 hrs. of call on a Saturday and 24 hours on a Sunday. Call includes some half nights (from 530 pm- 9 pm) and some 530 pm- 7 am call shifts. Must be prepared to arrive and be ready to work within 30 min. of STEMI activation. Holidays as needed. FIRST TIME TRAVELERS - OK! Minimum 2rs exp required. *ACLS BLS required. *Scrub REQ Knowledgeable of Orbital atherectomy Rotational atherectomy IABP IMPELLA OTC ECMO flow wire manifold Sheath pulling and all other Cath lab adjuncts. Will only ask to monitor case if proficient with XPER and EPICs CUPID. Scrubs provided.
