Bright Heart Hospice is a family-owned hospice provider proudly serving patients and families throughout Oklahoma. Based in the Tulsa area we are committed to delivering compassionate high-quality care through our team of experienced healthcare professionals. We are currently seeking a dedicated and compassionate Registered Nurse (RN) Case Manager to join our growing team.
The ideal candidate is caring dependable and passionate about providing exceptional patient-centered care. This role requires strong clinical judgment excellent communication skills and the ability to coordinate care effectively across multiple healthcare settings.
As an RN Case Manager you will play a vital role in coordinating and managing patient care throughout the healthcare journey. You will collaborate closely with patients families caregivers and interdisciplinary healthcare teams to ensure patients receive the appropriate services support and education needed to achieve optimal outcomes and maintain quality of life.
- Assess patient needs and develop individualized comprehensive care plans.
- Coordinate care with multidisciplinary healthcare teams to ensure continuity of services across various care settings.
- Monitor patient progress and update care plans based on ongoing assessments and changing needs.
- Educate patients and families regarding health management treatment options and available community resources.
- Advocate for patients by facilitating access to appropriate healthcare services and support systems.
- Maintain accurate and timely documentation in accordance with organizational policies and professional standards.
- Ensure compliance with HIPAA regulations and maintain strict patient confidentiality.
- Collaborate with providers and specialists in areas including acute care managed care ICU PICU trauma geriatrics and other specialties as needed.
- Provide compassionate nursing support while promoting a safe and supportive environment for patients and families.
- Current Registered Nurse (RN) license in the State of Oklahoma.
- Previous experience in case management or a related nursing field preferred but not required.
- Strong interpersonal and communication skills with the ability to build relationships with patients families and healthcare professionals.
- Knowledge of HIPAA regulations and commitment to patient privacy and confidentiality.
- Ability to work independently and manage multiple cases effectively.
