Job Overview

This is an opportunity for a dedicated Registered Nurse Case Manager to join our client and make a difference in the lives of patients in hospice care. You will be part of a supportive team providing essential care and coordination for patients and their families. Key Responsibilities Manage patient care in hospice settings Coordinate with healthcare teams Provide leadership and support to nursing staff Schedule and Shift Details Schedule: Monday Friday 8:00 am 6:00 pm 1 weekend per month and 2 holidays per year Travel Local travel required covering multiple counties.

Requirements

Qualifications

High School/GED

Hospice (1 years)
Management (1 years)

1 years of experience in ANY of the following:
- Home Health Care
- Epic Home Health
- Home Health

Valid Registered Nurse (RN) in the state of Pennsylvania (PA)
ALL of the following valid licenses/certifications:
- American Heart Association Basic Life Support (AHA BLS)
- American Heart Association Advanced Cardiac Life Support (AHA ACLS)

Have you applied to or worked for Residential Healthcare Group anytime in the last 6 months (no)
Are you okay with the required travel for this position (yes)

Required Skills:
