Pay Rate: $2400 to $2520/week
Contract: 13 weeks
Schedule: Day Shift
Guaranteed Hours: 36 hours per week

About the Role
We are seeking an experienced Home Health Registered Nurse Case Manager for a 13-week contract opportunity serving Vermont and New Hampshire communities. This role supports home health patients through assessment care planning case management patient education and coordination with physicians caregivers and interdisciplinary care teams.
Responsibilities
Screen evaluate re-evaluate and treat home health patients
Develop and update individualized care plans with physicians patients families and caregivers
Coordinate patient care through interdisciplinary communication and case conferences
Assess clinical functional and emotional status and report significant changes
Document evaluations treatment goals care updates and patient education accurately using EPIC
Requirements
Active Vermont And New Hampshire Registered Nurse License Or Compact RN License Required
Graduate of an accredited school of nursing required
Minimum 2 years of Home Health RN experience required
BLS/BCLS Certification Required
Strong case management care planning patient education documentation and interdisciplinary communication skills required
