Pay Rate: $1900 To $2120/Week
Contract: 13 weeks
Schedule: Monday to Friday 8:00 AM 5:00 PM
Experience: Minimum 2 years of Hospice RN experience required
About the Role
We are seeking a compassionate Primary RN Case Manager for a 13-week contract opportunity with HopeWest Hospice in Grand Junction Colorado. This role involves managing an assigned hospice caseload leading an interdisciplinary care team and providing exceptional end-of-life care in patients homes nursing homes assisted living facilities and other community settings.
Responsibilities
Manage an assigned caseload of hospice patients and coordinate individualized care plans
Provide skilled nursing care symptom management and patient assessments
Educate patients and caregivers on disease progression symptom management and end-of-life care
Collaborate with the interdisciplinary team to ensure quality patient-centered hospice care
Document patient care accurately and participate in occasional weekend backup and on-call coverage
Requirements
Active Registered Nurse (RN) License required
Minimum 2 years of Hospice RN experience required
Strong clinical assessment critical thinking and case management skills
Experience providing end-of-life and symptom management care
Excellent communication patient education and interdisciplinary collaboration skills
