What Youll Do

• Client Assessments & Care Plans Evaluate needs and design personalized care plans
• Coordinate Services Partner with nurses caregivers and providers for seamless support
• Advocate for Clients Be a trusted voice for patients and families
• Maintain Compliance Keep records accurate and services aligned with state and federal guidelines
• Crisis Support Step in quickly when urgent issues arise
• Monitor & Adjust Track progress and adapt care plans to improve outcomes

What Were Looking For

• Excellent communication & relationship-building skills
• Strong organization and time-management abilities
• Knowledge of Medicaid Medicare and Indiana home care guidelines
• Background in case management social work or healthcare
• Tech-savvy with EHR or case management systems
• Bilingual (English/Burmese) is a plusimmediate need!
