How Youll Make an Impact

- Complete in-home RN assessments; craft individualized Care Plans with client/family approval.

- Orient caregivers to each Care Plan perform field competency checks and ensure ongoing compliance.

- Track vitals condition changes and interventions in our HIPAA-secure EMR; escalate concerns promptly.

What Youll Bring
- Active Registered Nurse (RN) license in New Jersey
- Bachelors in Nursing or related human-services field OR 2 years geriatric/home-care experience
- Solid clinical assessment skills and a passion for problem-solving
- CPR/First Aid certification and a valid drivers license
- Warm professional communication that builds trust with families and field staff

Compensation: $50.00 per hour
