If you are ready to move beyond standard shift work and step into a holistic community-focused nursing career where your skills transform lives every day we want to meet you!Your Key ResponsibilitiesClinical Assessment & Care PlanningHolistic Evaluations: Conduct thorough medical assessments for incoming individuals partnering closely with the Intake team to ensure appropriate community Plan Leadership: Collaborate with physicians to develop implement and routinely update individualized service and nursing care plans that prioritize long-term health and Development: Generate robust healthcare protocols for specific medical conditions outlined in individual health and care Administration & CoordinationSafe Administration: Manage and administer medications in strict accordance with clinical guidelines ensuring meticulous documentation and compliant record -Risk Management: Oversee the clinical care of high-risk medical conditions and coordinate private duty nursing services as Liaison: Maintain seamless communication with primary care physicians and specialists accompanying individuals on medical visits when Mentorship & CollaborationEmpowering Others: Design tailored medical training tools and deliver clinical skills training to clients staff mentors respite providers and biological Excellence: Present vital medical skills components during Pre-service Orientation for new team Leadership: Actively contribute to Individual Health Plan (IHP) teams Human Rights Committees and Health & Safety administrative Support: Participate in an on-call system and assist with essential transitional or household coordinationsuch as safe transportation to appointmentsto guarantee continuity of You Bring to the TeamEducation & ExperienceDegree: Graduate of an accredited Registered Nurse (RN) : Minimum of one (1) year of experience working with medically complex Licenses and RegistrationsCurrent and valid State RN License in good CPR / First Aid drivers license active car registration and auto insurance (required for providing participant transportation).Physical Requirements & SafetyPhysical Strength: Heavy Work. This position requires exerting up to 100 pounds of force occasionally up to 50 pounds frequently and/or up to 20 pounds constantly to safely move objects and support individual Pathogens: This role involves tasks that carry a potential exposure to blood body fluids or tissues; standard clinical precautions and training are an expectation of : Ability to travel locally as needed to support community-based services.
