Position Overview

Expected Hire Date: November 2026

Hours per Week: 32 hours/week (28 clinical/programmatic 4 administrative) full-time at full salary. Will require 1-2 evening shifts per week.

note: Employee must complete an 8-week foundational onboarding period of 40 hours per week for a total of 320 hours before transitioning to 4Day work week (32 hours)

A Day in the Life of a Registered Dietitian with Winters Healthcare

If you were a Registered Dietitian Nutritionist with Winters Healthcare you would spend your day embedded within our integrated primary care teams. Patients may be introduced to you through a warm handoff from a physician physician assistant nurse practitioner behavioral health clinician dentist or population health coach. You might meet with a newly diagnosed patient with diabetes help a pregnant patient navigate healthy eating during pregnancy provide medical nutrition therapy for a patient with obesity and hypertension or assist a family experiencing food insecurity.

You will provide both individual and group services. Individual visits may include comprehensive nutrition assessments care plan development chronic disease management and follow-up counseling. Group services include a 10-week Nutrition and Lifestyle Medicine Program featuring nutrition education cooking demonstrations physical activity stress management goal setting and peer support. The RDN will oversee group programming and support curriculum development and quality improvement activities.

Beyond direct patient care you will work closely with medical providers behavioral health clinicians dental teams population health staff and community partners to improve access to nutritious foods. Summary of Duties:

Under the supervision of the Director of Clinical Services the Registered Dietitian (RD) serves as the clinical nutrition lead for individual and group-based nutrition services. This position provides medical nutrition therapy nutrition education and counseling services while partnering with interdisciplinary care teams to prevent and manage chronic conditions address food insecurity and improve health outcomes through integrated population health-focused approaches.

Responsibilities

Clinical Nutrition Services

• Provide individualized medical nutrition therapy for pediatric adult prenatal and geriatric patients.
• Conduct comprehensive nutrition assessments and develop individualized care plans.
• Manage nutrition care for patients with diabetes prediabetes hypertension obesity underweight cardiovascular disease pregnancy-related nutrition needs and food insecurity.
• Document care in Epic EHR and participate in team-based care planning.
• Use evidence-based interventions consistent with current nutrition practice standards.

Program Development and Leadership

• Assist in developing nutrition workflows protocols screening tools referral criteria and documentation standards.
• Design and maintain patient education materials care pathways and community resource directories.
• Assist in developing and overseeing nutrition-related clinical templates assessment tools and care plans.
• Serve as the organizations nutrition subject matter expert.
• Attend at least one nutrition-focused state or national conference per year.

Group Visits and Lifestyle Medicine

• Lead or oversee nutrition and lifestyle medicine group visits.
• Collaborate with the Population Health Coach to deliver a 10-week Nutrition and Lifestyle Medicine curriculum.
• Provide or oversee cooking demonstrations nutrition instruction and behavior-change counseling.
• Participate in community outreach and food-literacy activities.

Workforce Development

• Train staff on nutrition screening referral processes food insecurity interventions and evidence-based nutrition care.
• Conduct case-based education and interdisciplinary learning sessions.
• Participate in onboarding and continuing education for clinical teams.

Quality Improvement and Population Health

• Participate in quality improvement initiatives focused on nutrition-related outcomes.
• Assist with development of registries reports dashboards and outcome tracking.
• Monitor participation referral completion service utilization and clinical outcomes.
• Collaborate with leadership to analyze program performance and identify improvement opportunities.

Community Partnerships

• Collaborate with local food banks social service agencies community-supported agriculture programs and other nutrition-focused partners.
• Help patients access food resources and nutrition-related community services.
• Support food box produce prescription and healthy-food access initiatives.

Qualifications

• Bachelors Degree from the American Dietetic Association (ADA) accredited program.
• Registered Dietitian Nutritionist credential through the Commission on Dietetic Registration.
• Bilingual fluency in English and Spanish (speak read write)
• Eligibility to practice in California and ability to meet all applicable professional requirements before hire.
• Minimum of one year of experience providing nutrition counseling or medical nutrition therapy.
• Experience supporting chronic disease management including diabetes obesity hypertension or cardiovascular disease.
• Strong communication teaching motivational interviewing and relationship-building skills.
• Ability to work independently and as part of an interdisciplinary healthcare team.
• Commitment to serving rural and underserved communities.

Preferred

• Certified Diabetes Care and Education Specialist credential or interest in obtaining certification.
• Experience in primary care an FQHC community health public health or lifestyle medicine.
• Experience facilitating group visits group education or community nutrition programs.
• Experience with food insecurity programs or produce prescription initiatives.
• Familiarity with Epic EHR and population health workflows.

Compensation and Benefits:

• Annual equivalent salary range is $8100-$99000 for 32 hrs/week^^ and depends on experience and qualifications.

• Monday and Tuesday 8 am-5 pm
• Wednesday and Thursday 11 am-8pm

Winters California and Esparto California.

Some community outreach activities may occur at partner locations throughout Yolo County.
