As a Registered Dietitian at PACE of Southwest Michigan youll help older adults maintain their health independence and quality of life through personalized nutrition care. Working alongside medical nursing rehabilitation and social services professionals youll develop individualized care plans and play a vital role in supporting participants overall well-being.
Key Responsibilities
- Conduct comprehensive nutrition assessments upon enrollment semi and annually and with any significant change in participant condition.
- Develop implement and monitor individualized nutrition care plans.
- Provide medical nutrition therapy (MNT) for chronic diseases.
- Monitor weight trends laboratory values oral intake dentition and nutritional risk indicators.
- Recommend therapeutic diets oral nutrition supplements and texturemodified diets.
- Actively participate in IDT meetings care planning and case conferences.
- Provide individualized nutrition education and counseling to participants and caregivers.
- Conduct home visits as needed to assess food access safety and ability to prepare meals.
- Coordinate with home care teams to address nutritionrelated barriers in the community setting.
- Organize home delivered meals and supplements as required.
- Oversee the PACE kitchen area.
- Assist in monitoring meals including intake diet compliance and tray accuracy.

Skills Knowledge & Expertise
- Bachelors or Masters degree in Nutrition Dietetics or related field.
- Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential.
- Minimum of 12 years of clinical nutrition experience (geriatrics longterm care or community health preferred).
- Minimum of one (1) year working with the frail and elderly.
- Certified Diabetes Educator (CDE) desired.
- ServSafe Food Protection Manager certification required - if certification is not held at the time of hire it must be obtained within six months of the hire date.
- Valid drivers license and reliable transportation.
