The Registered Dietitian is responsible for implementing an individualized nutritional care plan for each patient. This role is also responsible for developing or obtaining patient nutrition educations materials. The Registered Dietitian counsels the patient family significant other or caregiver on prescribed diet and monitor adherence and response to therapy.

- Co Develops implements and monitors the clinical management of parenteral and enteral patients as part of the Nutrition Support Team
- Makes recommendations for nutrition intervention when necessary
- Reviews patient medical history anthropometrics and pertinent lab values to assess home nutrition support protocols and insurance payer requirements
- Counsel patients and caregivers on home enteral and parenteral nutrition therapy. Assesses patients understanding of and compliance with prescribed diet principles
- Ensure continuity of care with patients physician nurses and nutrition support teams to provide high quality patient centered care
- Participate in Performance Improvement Program and ACHC accreditation
- Identifies and promotes nutrition program services to clinical and professional referral base and performs necessary in-service education
- Supervisory Responsibility: No
- Bachelor of Science in Nutrition or related field
- Registered Dietitian with license to practice in assigned state(s); Certified Nutrition Support Clinician preferred
- Minimum of two (2) years of clinical experience required.
- Must have strong communication skills and organizational skills
- High level of self-motivation initiative flexibility and ability to work under pressure
**To perform this role will require constant sitting standing walking bending reaching and typing on a keyboard with fingers. The physical requirements will be the ability to push/pull and lift/carry 10-30 lbs**
