Job Summary

Schedule: Full Time 40 hours per week Monday - Friday 8:00am - 4:30pm
Department: Admin Muskegon and Holland MI

What Youll Do In This Role:
As a Referral Coordinator youll help connect participants to the care they need by coordinating referrals scheduling appointments and managing authorizations. Youll deliver a seamless positive experience and support exceptional participant care.

Key Responsibilities

Scheduling and Coordination
  • Schedule participant medical appointments coordinate transportation and ensure necessary authorizations are obtained for services.
  • Document appointments in the electronic health record (EHR) notifying the interdisciplinary team (IDT) and clinical teams as needed and ensure authorizations are in place.
Liaison and Communication
  • Serve as the liaison between the IDT clinic team and transportation department for outside appointments.
  • Communicate effectively with participants families and caregivers about appointments transportation and care details.
Compliance and Documentation
  • Ensure participant information is documented accurately and in compliance with guidelines and regulatory requirements.
  • Communicate with referral sources and funding sources as needed for coordination of care.
Collaboration and Administrative Support
  • Work with the Health Plan Management Department and other LifeCircles teams to ensure coordinated care.
  • Perform clinical administrative tasks including managing calls processing faxes and handling referrals.
Technical Proficiency and Additional Duties
  • Use computer software correspondence tools and telecommunication systems to manage tasks efficiently.
  • Perform other assignments as directed by the supervisor.

Skills Knowledge & Expertise

  • Experience in scheduling referral coordination and/or medical administrative duties preferred.
  • Experience with electronic medical records
  • High School Diploma or GED required
  • CPR certification or willing to obtain upon hire
  • Experience in working with the elderly preferred.
  • Must clearly demonstrate a high degree of organizational skills multi-tasking and efficiency.
