The Referral Coordinator is responsible for managing and coordinating all internal and external patient referrals across service lines to ensure timely accurate and seamless access to care. This role plays a critical part in supporting a centralized referral management model that enhances patient experience continuity of care and operational efficiency.

Serving as a key liaison between patients providers internal care teams and external partners the Referral Coordinator ensures referrals are processed tracked and closed appropriately within the Electronic Health Record (EHR) in alignment with organizational standards and payer requirements.

  • Receives reviews and processes all incoming referrals (internal and external) ensuring completeness accuracy and timely documentation in the Electronic Health Record (HER).
  • Verifies required information authorizations and supporting documentation prior to processing.
  • Coordinates referral appointments across internal departments and external providers.
  • Facilitates scheduling documentation and communication to ensure continuity of care.
  • Serves as a point of contact for patients and care teams to navigate referral-related requests.
  • Manages the full life cycle of referrals using a closed-loop process.
  • Tracks referral status confirms appointment completion and obtains consultation results or documentation.
  • Proactively follows up on pending or incomplete referrals to remove barriers to care.
  • Partners with clinical and operational leaders to improve referral workflows and system efficiency.
  • Identifies trends gaps and opportunities to enhance patient access and service delivery. Contributes to process standardization and best practices across service lines.

Qualifications
  • High School diploma or GED required.
  • Medical Assistant (MA) Behavioral Health Technician (BHT) or equivalent clinical experience strongly preferred.
  • 13 years of experience in a healthcare setting preferably in referral coordination scheduling patient access or care coordination.
  • Proficiency with EHR systems (NexGen preferred) and Microsoft Office applications
  • Strong organizational skills with the ability to manage multiple priorities in a fast-paced environment.
  • Must have a valid Arizona drivers license be 21 years of age with a minimum of 3 years driving experience and meet requirements of Terros Healths driving policy
  • Must successfully pass a TB screening and criminal background check
