Recovery Coach (Overnight) Residential Step Down

This is a union-eligible expected schedule for this role: overnight shiftJOB SUMMARYHealthRIGHT 360 a nonprofit organization and a family of programs is committed to providing accessible and comprehensive healthcare services to vulnerable populations. Our mission is to tackle systemic barriers to healthcare and promote health equity for all. We offer a wide range of services including mental health care residential and outpatient substance use treatment and primary health services. Additionally we provide transitional support for individuals re-entering the community after involvement in the criminal justice system. By integrating physical and behavioral health we empower individuals to overcome challenges by addressing social determinants of health fostering resilience and facilitating Treasure Island Residential Step Down (RSD) services are available to beneficiaries who are stepping down from inpatient/residential substance use disorder treatment. Clients must be concurrently in treatment specifically in outpatient (OP) intensive outpatient (IOP) Opioid Treatment Program (OTP) or Outpatient (aka Ambulatory) Withdrawal Management (OP-WM) settings. RSD provides a temporary drug and alcohol-free environment to residents that are actively engaged in outpatient treatment for medically necessary SUD provided to the client Recovery Coach is responsible for assessing client needs and strengths in support of health and recovery. Through individual coaching with program-specific requirements and case management Recovery Coaches assists clients in navigating systems of care while providing a supportive recovery environment. Recovery Coaches are responsible for maintaining and assessing the safety and security of the program and its residents through continued house runs while providing a supportive environment. Through effective communication Recovery Coaches relay relevant information to other members of the interdisciplinary team and perform crisis intervention as needed. Recovery Coaches ensure that the facility environment is safe and RESPONSIBILITIESClient Services Welcomes and assists clients with navigating throughout the excellent customer service and addresses any immediate needs of patients with required paperwork as needed for proper enrollment in transportation for patients in residential Responsibilities: Provide direct services such as intakes assessment crisis intervention and case management as for retention and completion of clients facilitate individual coaching sessions with each caseload learning experience opportunities and offers support to assist clients in meeting their long-term recovery link clients to both internal and external resources based on their recovery needs and follows up on the progress/ house runs to ensure and maintain accountability of all clients and the safety and security of the crisis intervention and communicates with treatment team as unforeseen situations client updates and incidents in the facility log daily performs CPR and First Aid including administering Narcan as needed and work weekends and holidays as Setting Responsibilities: Facilitate community meetings and ensures compliance with facility crisis intervention and communicates with team as unforeseen situations all client updates and facility incidents daily in the facility periodic house runs to ensure and maintain the safety and security of the assigned at the Health and Wellness office the Recovery Coach will document and accurately distribute client money ensure client medications are securely stored and properly accounted for and holds facility keys and floater cellular needed accompanies clients to offsite appointments related to their all required trainings and meetings as assigned by facility and facilitate client celebrations and special Coach must be available to work weekends and holidays as Responsibilities: Collaborate with each client and other available internal and external resources to develop transition plans progress notes and appropriate updates in support of the health and recovery needs of the document all individual encounters and completes the discharge paperwork/process and required agency assessments in timely accurate records by data entering documentation into various electronic systems for all clients in accordance with guidelines established by HealthRIGHT 360 to satisfy internal and external evaluating accountability report on a weekly basis and collaborates with treatment providers to assist clients achieve program Responsibilities Remains alert and awake during the entire and maintains a safe and orderly environment for clients staff and crisis intervention and communicates with treatment team as unforeseen situations and deflects traffic; de-escalates tense high pressure and emotionally charged situations; and fosters a safe respectful and welcoming positive respectful non-confrontational behaviors always even when observing receiving or de-escalating disrespectful or disorderly regular rounds to monitor entrances exits offices common areas and building a welcoming environment by clearing areas of crowds debris and out-of-place collaboratively with service providers and administrative staff to maintain an orderly CPR and First Aid as needed. May work weekends and holidays as perform other duties as Certification and Experience High School diploma or Aid and CPR certification.A valid California drivers license and automobile working successfully with issues of substance abuse mental health criminal background and other potential barriers to economic self-sufficiency.A Drug and Alcohol Registration/Certification recognized by DHCS is degree in related field is Knowledge of co-occurring disorders and trauma informed of Clinical documentation (treatment plans progress notes etc.).Preferably bilingual in San Francisco threshold of gender-responsive trauma informed and co-occurring treatment.
