Recovery Coach (On-Call) Residential Step Down

This is a union eligible SUMMARYHealthRIGHT 360 a nonprofit organization and a family of programs is committed to providing accessibleand comprehensive healthcare services to vulnerable populations. Our mission is to tackle systemicbarriers to healthcare and promote health equity for all. We offer a wide range of services includingmental health care residential and outpatient substance use treatment and primary health we provide transitional support for individuals re-entering the community after involvementin the criminal justice system. By integrating physical and behavioral health we empower individuals toovercome challenges by addressing social determinants of health fostering resilience and Treasure Island Residential Step Down (RSD) services are available to beneficiaries who are steppingdown from inpatient/residential substance use disorder treatment. Clients must be concurrently intreatment specifically in outpatient (OP) intensive outpatient (IOP) Opioid Treatment Program (OTP) orOutpatient (aka Ambulatory) Withdrawal Management (OP-WM) settings. RSD provides a temporarydrug and alcohol-free environment to residents that are actively engaged in outpatient treatment formedically necessary SUD provided to the client On-Call Recovery Coach is responsible for assessing client needs and strengths in support of healthand recovery. Through individual coaching with program-specific requirements and case managementOn-Call Recovery Coaches assists clients in navigating systems of care while providing a supportiverecovery environment. On-Call Recovery Coaches are responsible for maintaining and assessing the safetyand security of the program and its residents through continued house runs while providing a supportiveenvironment. Through effective communication On-Call Recovery Coaches relay relevant information toother members of the interdisciplinary team and perform crisis intervention as needed. On-Call RecoveryCoaches ensure that the facility environment is safe and RESPONSIBILITIESClient Services: Welcomes and assists clients with navigating throughout the building. Provides excellent customer service and addresses any immediate needs of patients. Assists patients with the required paperwork as needed for proper enrollment in services. Coordinates transportation for patients in residential Responsibilities: Provide direct services such as intakes assessment crisis intervention and case management asneeded. Responsible for retention and completion of clients facilitate individual coaching sessions witheach caseload client. Provide learning experience opportunities and offers support to assist clients in meeting theirlong-term recovery goals. Proactively link clients to both internal and external resources based on their recovery needs andfollows up on the progress/status. Perform house runs to ensure and maintain accountability of all clients and the safety and securityof the facility. Perform crisis intervention and communicate with treatment team as unforeseen situations arise. Document client updates and incidents in the facility log daily perform CPR and First Aid includingadministering Narcan as needed and work weekends and holidays as Setting Responsibilities: Facilitate community meetings and ensure compliance with facility rules. Perform crisis intervention and communicate with team as unforeseen situations arise. Document all client updates and facility incidents daily in the facility log. Perform periodic house runs to ensure and maintain the safety and security of the facility. When assigned at the Health and Wellness office the Recovery Coach will document andaccurately distribute client money ensure client medications are securely stored and properlyaccounted for and holds facility keys and floater cellular phones. As needed accompanying clients to offsite appointments related to their recovery. Attending all required training and meetings as assigned by facility manager. Plan and facilitate client celebrations and special events. Recovery Coach must be available to work weekends and holidays as Responsibilities: Collaborate with each client and other available internal and external resources to developtransition plans progress notes and appropriate updates in support of the health and recoveryneeds of the client. Properly document all individual encounters and complete the discharge paperwork/process andrequired agency assessments in timely manner. Maintains accurate records by data entering documentation into various electronic systems forall clients in accordance with guidelines established by HealthRIGHT 360 to satisfy internal andexternal evaluating requirements. Completes accountability report on a weekly basis and collaborates with treatment providers toassist clients achieve program Responsibilities Remains alert and awake during the entire shift. Ensures and maintains a safe and orderly environment for clients staff and visitors. Performs crisis intervention and communicates with treatment team as unforeseen situationsarise. Directs and deflects traffic; de-escalates tense high pressure and emotionally charged situations;and fosters a safe respectful and welcoming environment. Maintains positive respectful non-confrontational behaviors always even when observingreceiving or de-escalating disrespectful or disorderly behaviors. Make regular rounds to monitor entrances exits offices common areas and building perimeters. Ensure a welcoming environment by clearing areas of crowds debris and out-of-place items. Work collaboratively with service providers and administrative staff to maintain an orderlyenvironment. Performs CPR and First Aid as needed. May work weekends and holidays as perform other duties as Certification and Experience High School diploma or equivalent. First Aid and CPR certification. A valid California drivers license and automobile insurance. Experience working successfully with issues of substance abuse mental health criminalbackground and other potential barriers to economic self-sufficiency. A Drug and Alcohol Registration/Certification recognized by DHCS is preferred. Bachelors degree in related field is Knowledge of co-occurring disorders and trauma informed treatment. Knowledge of Clinical documentation (treatment plans progress notes etc.). Preferably bilingual in San Francisco threshold languages. Knowledge of gender-responsive trauma informed and co-occurring treatment.
