While your work as a member of the CST is crucial to improving the mental health and overall well-being of our clients our organization also offers services in education employment support residential programs and community engagement. • Provide direct support and guidance to individuals in the community who are experiencing mental health or substance use challenges
• Meet clients where they areat home in the community or wherever support is needed
• Develop and maintain strong trusting relationships with clients
• Collaborate with interdisciplinary teams to create person-centered care plans
• Document interactions progress and outcomes in accordance with agency policies and regulations
• Advocate for clients to ensure access to resources and services

• Bachelors degree in human services social work or a related field preferred
• Experience working with individuals experiencing mental health or substance use challenges is highly desirable
• Excellent communication problem-solving and interpersonal skills
• Ability to work independently in community settings
• Valid drivers license and reliable transportation required
