Were seeking a compassionate organized and mission-driven Qualified Intellectual Disabilities Professional Case Manager to support adults with developmental disabilities in living full meaningful lives.

• Build relationships with clients and families ensuring services reflect each persons unique needs and wishes.
  
• Coordinate service agreements authorizations and referrals.
  
• Develop and implement strategies in collaboration with families and support teams.
  
• Maintain accurate electronic records (Quantum) and case notes.
  
• Provide advocacy and support during crises.
  
• Partner with community resources and internal teams to ensure holistic care.
  
• Travel throughout Cook County for client visits (about 70% in the community).
  

• At least 1 year working with adults with developmental disabilities.
• Bachelors degree in Human services field including but not limited to Sociology Special Education Rehabilitation Counseling or Psychology Recreation or in a specialty area such as Art Dance Music or Physical Therapies.

Also required:

• Valid drivers license and reliable vehicle.
