As a PT Social Worker youll partner closely with the SW/CM to drive strong resident outcomes by supporting each persons physical emotional and psychosocial well-being. This role exists to keep transitions smooth communication clear and resident rights front and centerevery single day.

Key responsibilities include assisting with discharge planning coordinating communication between residents families/representatives and the interdisciplinary team and advocating for resident rights in line with Fellowship standards.

Required qualifications/skills:
- Bachelors degree in Social Work
- Minimum two years experience in geriatric care
- Excellent interpersonal/public relations skills; strong organization
- Ability to counsel individuals in crisis; knowledge of senior community resources
- Intermediate computer skills; working knowledge of the MDS
- Understanding of Medicare/MA/managed care/private pay basics; medical terminology
